This Vancouver area patient in his mid-30’s came to the clinic to address his receded hairline and thinning frontal zone. He wasn’t sure whether he wanted his new hairline to have a more conservative “V” like design or a more aggressive design which was a little more linear. After further thought and discussion with Dr. Hasson, the more aggressive design was the choice. Given his donor area characteristics and projected final hair loss pattern, the more aggressive approach would be possible and not leave him in an unenviable position in the future. Facial features and bone structure are taken into consideration and a few options regarding the design of hairline are usually possible. There are head shapes and features that would dictate the best hairline to actually make a patient look more handsome (or attractive, if you will, in the case of a female patient). Dr. Hasson is very good at assessing what design may best suit each patient. This skill can often times be overlooked as a critical component of an impressive and natural looking hair transplant. All other aspects can be in place but without proper hairline design, the aesthetics of the result will suffer. In some cases a patient will choose a hairline that will still look good and allow for a significant cosmetic improvement that would not be the hairline Dr. Hasson would choose. There must be a balance between what the patient would like and what will still look natural. While there may be two to three choices and the design the patient choses may not necessarily be the design Dr. Hasson would choose, there is some leeway. However, he will not recreate anything that will look unnatural regardless of what the patient may desire. A patient is not likely to have a total understating of hairline design and it is for this reason Dr. Hasson must protect the patient from himself in some cases when an inappropriate hairline is requested.
Once the hairline was determined, Dr. Hasson and team used 2575 grafts extracted via FUE to restore the area of loss to achieve the desired result. Breaking down the 2575 grafts revealed 302 were single hair grafts, 2017 were doubles and 256 were 3-4 hair grafts. The ability to choose only the grafts desired via the FUE method of extraction allowed for a larger number of double hair grafts to be employed than would usually be available with FUT. This allows Dr. Hasson to achieve a higher density in the frontal zone behind the hairline with fewer grafts where larger double and 3-4 hair grafts can be used. Only a limited number of single hair grafts are needed to create the most natural looking hairline and in this case only the exact number Dr. Hasson feels would be sufficient will be harvested. This is one of the reasons smaller to medium sized procedures that don’t require restoring a very large area can be better suited for FUE. These photos were taken just shy of 1 year post-op. As you may have guessed, both Dr. Hasson and the patient were very happy with the outcome of this hair restoration procedure in which final hairline design of course played a key role. *At the time of his procedure, we were experimenting with a white background and we have since gone back to blue. Hence the background color difference in the before versus the after photos.

If the laxity and quality of the donor was less than ideal and the yield would have come in at closer to 4,000 to 4,500, a smaller second hair transplant procedure to address the crown would have been required. For some patients whose donor hair is average or less and who have a larger area to cover, the positive impact to the appearance can be similar but crown coverage may not be possible. The game plan was to reconstruct a new hairline, frontal zone and mid-scalp working back as far as possible. With the anticipated yield of something close to the 5,000 graft range, some crown coverage would be possible. Dr. Wong and team of hair transplant technicians transplanted 5496 Grafts in one session via FUT. A look at the breakdown of the 5496 grafts reveals 1562 were single hair grafts, 3317 were double and 617 were 3-4 hair grafts. With a final graft count above 5,000, as you can see, a lighter coverage of the crown was achieved in this case. The result shown is only 7.5 months post-op. This patient is very happy and a strong advocate for Hasson and Wong.
